# Practice credentialing conversation

**Discussion checklist for owner review · 2 October 2026**

Use this list to discuss the documentation relevant to the assigned clinician, your office and the proposed case. It is not proof of credentials, an insurance certificate, a permit or a clinical clearance.

## Confirm the clinician

- Confirm the name and role of the clinician assigned to the treatment day.
- Ask which professional qualifications and board-certification documentation are available for review.

## Confirm location-specific requirements

- Discuss the licensure and permits applicable to the clinician and treatment location.
- Confirm which host-office requirements and readiness checks apply before scheduling.

## Review the working arrangements

- Ask about relevant insurance and credentialing documentation.
- Agree on equipment responsibilities, office preparation, emergency planning and team roles.
- Confirm how documentation will be provided, who will review it and how updates will be communicated.

## Keep clinical records separate

The anesthesia record documents the care provided. Credentialing documents serve a different purpose. Patient records should use the approved secure clinical process, not the public website or a general practice inquiry.
