Practice-planning draft for clinical review · 2 October 2026

Use this checklist to prepare a conversation with Aspire. This is not office clearance or an approved technical requirements sheet. Exact requirements, provider and office responsibilities, and case suitability must be confirmed before scheduling.

Practice and proposed date

  • Confirm the practice address, main contact and requested dates.
  • Confirm service-area availability and the requirements applicable to the clinician and host office.

Room, access and recovery

  • Review building access, stairs or elevator, the equipment route and working space around the dental chair.
  • Identify the proposed recovery area and staffing arrangements for review.

Equipment, utilities and emergency arrangements

  • Confirm the case-specific anesthesia workstation, monitoring, airway and emergency equipment.
  • Document who supplies oxygen, suction, electrical access and other required infrastructure.
  • Review emergency roles, communication and transfer arrangements with the clinical team.

Team, preparation and records

  • Agree on dental staffing, treatment duration, arrival and setup arrangements, and patient coordination.
  • Use Aspire’s approved secure intake process for patient history and clinical documents.
  • Confirm who provides preparation instructions, discharge information and follow-up communication.

Fees and confirmation

  • Clarify anesthesia fees, payment arrangements, cancellation terms and separate dental and anesthesia billing.
  • Confirm case review, office readiness and scheduling status directly with Aspire. An inquiry does not establish acceptance or a booking.

Do not enter patient information into this document. Specific measurements, inventory, staffing, timing, billing terms and jurisdictional requirements require approval before a final requirements sheet is issued.
